What is Individual Community Living Support?

Independence is not about doing everything alone. It is about having the right support to live the life you choose. Our individual community living support helps people with disabilities and seniors engage with their communities on their own terms. We assist with social activities, volunteering, recreation, community events, and building the daily living skills that make independence possible. Our goal is not to do things for your loved one but to help them do things for themselves, with support that fades into the background as confidence grows.

How services are funded

Most home care services in Minnesota are covered through public funding programs. You do not need to pay out of pocket to receive quality care at home.

What kind of community activities do you support?
Whatever matters to your loved one. That might include grocery shopping, visiting the library, attending community events, going to the gym, volunteering, joining a class, or simply spending time at a favorite park. The activities are driven by their interests and goals.
How is this different from companion services?
Community living support is more goal-oriented. While companion services focus on social connection and engagement, community living support actively builds independence skills. The caregiver is a coach and facilitator, helping your loved one develop the abilities they need to participate in community life on their own terms.
Does this service include transportation?
Yes. Our caregivers provide transportation to and from community activities as part of the service. Getting there is often the biggest barrier to community participation, and we make sure it is not.
Who develops the community living support plan?
The plan is developed collaboratively with your loved one, their family, their case manager, and our care team. It is based on their person-centered plan and focuses on the goals that matter most to them.
