Started by people who saw firsthand what families go through

We Care Home Health was founded in Minnesota with a simple observation: too many families struggle to find reliable, compassionate home care when they need it most. The process is confusing, timelines are slow, and the people who show up are often strangers rotating through a schedule.

We started this agency to do things differently. Every decision we make — from how we hire caregivers to how quickly we respond to referrals — is guided by one question: would this be good enough for our own family?

Today, we serve over 150 families across the Twin Cities metro and surrounding counties. We are licensed by the Minnesota Department of Human Services under the 245D framework, and we work closely with case managers, county social workers, and families to ensure the right support is in place — and stays in place.

We are not the biggest agency in Minnesota, and that is by design. Staying focused means we can be responsive, consistent, and personal in a way that larger organizations simply cannot.

When a family needs help, waiting weeks is not an option. Most referrals begin services within 72 hours because we know how much every day counts.

Licensed and accountable

We Care Home Health is licensed by the Minnesota Department of Human Services under the 245D Home and Community-Based Services standards. We accept MA Waiver, CADI, BI, DD, EW, and CAC programs — and work directly with county case managers to coordinate care.
