Most of the care we provide is funded through Minnesota programs. Here is how our services are licensed and which programs pay for them.
245D Licensed Services
Minnesota's 245D licensing ensures that providers of home and community-based services meet rigorous standards for quality, safety, and person-centered care. We Care Home Health MN holds a 245D license, which means we have been evaluated and approved by the Minnesota Department of Human Services to provide disability services. This license covers our community living support, individualized home supports, and respite care services. For families, a 245D license is your assurance that the provider has trained staff, proper supervision, individual service plans, and accountability measures in place.
Medical Assistance Waiver Programs
Minnesota's Medical Assistance waiver programs help eligible people receive support at home and in their communities. Covered services vary by program and the person's assessed needs. Your case manager can explain the authorized support, and We Care can discuss whether we can provide it. Ask about any financial responsibility before agreeing to services.
